Autoposy was performed. [Note: This was performed on 3/23/61 by Ludolph H. van der Hoeven, M. D. Pathologist, assisted by Dr. Sherbourne. The clinical diagnosis was "Cerebral vascular accident." The autoposy report was found among Hazel's estate materials.]

Her attending physician from about 1951 to 3/22/61 was Dr. Shebourne 336 E. State Street. He signed the death certificate on 3/23/61

Burial was on 3/24/61 at Union Cemetery, Columbus, Ohio
